Hi my name is Tom Bartram, I'm an A&E consultant here at Macclesfield Hospital in

East Cheshire. Look the NHS is an amazing resource and you may not have to use it

yet but it's always there for us in our times of need. In recent years you may

have heard about the increasing pressures on healthcare services and how

we struggle particularly in the winter months to cope with the number of people

that need us. We want to be able to give everyone the best care but that can be

difficult when there are so many patients needing our services at the

same time. On average we admit 30 patients a day to the 200 beds we have

to treat patients but what happens when these beds are full and the flow of

patients just slows right down. If we can't discharge patients when they

are medically well or they don't leave hospital until late in the day what

happens to all those poorly patients who are waiting and A&E who really need a

bed. So how do we fix it there are no easy answers but by working together we

can make a difference we are constantly improving the way we do things here at

the hospital to make things better but what can you do to help Flo.

Admission to hospital is not always the first, only or best option you may be referred to

hospital by your own GP and you may even be transported in an ambulance like this

that doesn't necessarily mean that you will be admitted to hospital. Following

tests and investigations if it's safe to do so you will be discharged home. That

might be with an outpatient or clinic appointment or even a referral back to

your GP that's helping Flo. If you are admitted bring day clothes and the right

footwear as being out of bed can help recovery. It's been really important to

me to be able to get up out of bed, just starting to feel so normal again to get dressed

in your own clothes to be able to walk around when you have been used to being so fit and active. So if

you are admitted to hospital remember to bring day clothes and

footwear so that you can get up and keep moving.

If you need other things to stay active to such as glasses and hearing aids bring

them to. It really might mean that you could go home that little bit sooner

that's helping Flo. Please make sure all practical

arrangements are in place for your discharge. You might need someone to

collect you and take you home or stay with you for a while when you do go home.

You might need to ask us to organise transport or you might need us to look

into ongoing care for you. Making preparations will help us to get you

home as soon as you are ready that's helping Flo. Please prioritise

collecting your loved one as soon as they are ready

our aim is to discharge patients by 11:00 a.m. Freeing up beds for patients

that are acutely ill is one of the most important things we can do to make sure

we have good flow. If you are collecting someone from hospital it really does

make a difference if you can pick them up as soon as they are ready. If you can't be

picked up straight away we will probably bring you to our discharge lounge - an

area in the hospital where patients can wait to be collected or wait for

transport or discharging medication. Freeing up beds early in a day that's helping Flo.

Once you no longer need an acute hospital bed please be prepared to

receive any ongoing care in a different setting.

We have interim placements in various locations such as Macclesfield, Congleton, Knutsford and Wilmslow.

That's where we can continue to work with, you build up your

confidence, maximise your independence and hopefully get you straight home.

If you decline a rehabilitation bed because it's not as close to home as you

would like this can cause a delay for your rehab needs as well as for

the patients in A&E that require a hospital bed. We ask you

to consider them when it's time for discharge or transfer to another unit

this is helping Flo. This winter will certainly be another challenge for

healthcare across the country. We want you to get the best care in the

right place. Patients, carers and relatives can all help us keep hospital beds free

for the sickest patients by helping each other

Right Before Senate Hearing, Polygraph Of Kavanaugh's Accuser Is Made Public - Duration: 2:15.

Christine Blasey Ford's attorneys released the results from a polygraph test she took

last month.

Ford has accused Judge Brett Kavanaugh of sexually assaulting her over three decades

ago while they were both in high school.

She apparently passed the released polygraph test.

The exam was given to her by a former FBI agent on August 7th at a Hilton Hotel in Maryland,

according to Ford's lawyers.

It included two questions and an interview focused on the alleged assault.

The questions were: "Is any part of your statement false?"

And, "Did you make up any part of your statement?"

She simply answered "no" to both questions.

According to the results of the test, Ford gave answers "not indicative of deception"

and passed the test with "unremarkable" scores, according to her attorneys.

The results have been sent to members of the Senate Judiciary Committee and will likely

be brought up on Thursday during the Kavanaugh hearing.

One inconsistency in her testimony with the polygraph test that may be brought up at the

Senate Judiciary Committee is that she said there were "4 boys and a couple of girls

at the party."

In the written statement she originally sent to Sen. Dianne Feinstein (D- Calif.) and in

her interview with the Washington Post, she only mentioned "four others," or "four

boys at the party."

The polygraph test for Ford was administered just one week after she sent her sexual assault

allegations in a letter to Feinstein.

She said that Kavanaugh pinned her down on a bed and groped, then covered her mouth when

she tried to scream.

The Supreme Court nominee has denied any wrongdoing.

Critics attack ''public charge rule'' that would make it harder for people to get visas or green cards if they have used public benefits

US immigrants have begun to remove themselves from public housing waiting lists, childcare subsidy programs and the food stamp program - out of fears that a rule proposed on Saturday night could jeopardize their immigration status.

The Department of Homeland Security''s (DHS) "public charge rule" would make it more difficult for people to qualify for green cards and other visas if they use or have used public benefits. Immigrants began purging themselves from public assistance programs when a draft version of the rule leaked earlier this year.

"No matter what the final rule ends up being, in a lot of ways since February, the damage has already been done," said Carlyn Cowen of the Chinese American Planning Council (CPC) in New York City, a social services agency that helps 60,000 New Yorkers every year.

Cowen said that to offset the costs from losing benefits, people are taking additional jobs, either in the formal or informal job market, and moving in with other families.

"It''s forcing families to choose between risking the immigration status of them and their loved ones or meeting their basic needs," she said.

A "public charge" is a person who is dependent on a certain set of public benefits. The Trump administration''s rule would dramatically expand the definition to include people using, or who the government determines may one day use, a wider scope of benefits - even if they are not completely dependent on those services.

In the 450-page proposed rule, the DHS said factors that could negatively affect someone''s visa application include being younger than 18 or older than 61, having a medical condition that is likely to require extensive medical treatment, or a lack of a high school diploma or higher education.

The department said it would weigh the "totality of the circumstances" and meeting one of the factors would not necessarily block someone from getting a visa.

But critics, including doctors, said they opposed the policy because it targets poorer and unemployed Americans and those with health problems.

Darrell Kirch , the president of the Association of American Medical Colleges, said: "We urge the Department of Homeland Security not to finalize this rule that seeks to overturn decades of settled policy."

Alison Hirsh of the service union 32BJ SEIU, which has 163,000 members, said: "The proposed ''public charge'' rule is a shameful attempt to punish working-class immigrants for accessing the healthcare, housing and nutrition programs they need to support their families and survive in our country."

Douglas Rand, who worked on immigration policy in the Obama White House, said the rule was "a breathtakingly ambitious attempt to reshape the legal immigration system without an act of Congress".

He said: "Never in our history have we in this country said you have to be middle class to be an American. Never have we ever said you have to be in perfect health, you have to have a completely spotless credit history, you have to be working age - these are all huge departures from precedent."

Rand, who co-founded Boundless Immigration, a technology company that helps clarify the immigration process, said the proposal was filled with small details "designed to ensnare hundreds of thousands if not millions of people who would otherwise have been perfectly eligible for green cards or other visas".

The expansion would go as far as to penalize people for using visa fee waivers, which are prominently advertised on US government sites for people seeking information on the immigration process. It could also force 200,000 married couples each year to either leave the US or live apart indefinitely, according to Boundless.

"A very large number of people who meet in the US and fall in love, won''t be able to stay together," Rand said.

Before the rule can become official, it must be published in the federal register. It will then face a 60-day public comment period. Critics plan to flood the agency tasked with reviewing the rule, the Office of Management and Budget (OMB), with public comments it is legally required to sift through.

Cowen said the CPC alone plans for its 4,000-person staff, as well as its supporters, to write individual letters challenging the proposal.

"This administration has been so wildly unpredictable, and the only thing that really is predictable is that they are targeting immigrants and trying to separate immigrant families," she said. "So it''s difficult for community organizations to balance - how do we tell people make sure your family stays healthy because right now your benefits aren''t at risk with the potential future risk?"
